• Claim Status and copies of Explanation of Benefits (EOB) for claims processed
• Claim forms
• Discounts offered by Anthem

Access the Anthem website to find a provider:

• If not signing in, search National BlueCard Directory, then insert the 3 letters at the beginning of your ID number (Identification Prefix).
• If registering first, look for a provider “outside of Ohio”. You will then be asked for the 3 letters (Identification Prefix).

The above would also apply when traveling or dependents living out of State.
